Request to End Subscription toTexas Co-op Power and Waiver of Official Notices

  1. I understand the standard policy of Medina Electric Cooperative, Inc. (“MEC”) is to automatically provide to members of MEC a complimentary subscription to Texas Co-op Power, a magazine published monthly by Texas Electric Cooperatives.
  2. I further understand that MEC uses Texas Co-op Power for membership communications such as, but not limited to, the following: (1) providing official notices of various meetings of MEC, (2) providing notices of various changes to MEC’s tariffs, usage fees, MEC policies, and other information, and (3) educating MEC members on a variety topics, such as electrical safety.
  3. Notwithstanding the above, I desire MEC terminate my subscription to Texas Co-op Power. Accordingly, by executing below, I hereby request and direct MEC to terminate my subscription to Texas Co-op Power. I understand that once my subscription is terminated, MEC will be under no obligation to reinstate my subscription.
  4. 4. By executing below, I hereby absolutely and forever voluntarily waive any and all rights to receive any and all official notices and communications to MEC’s members. By executing below, I further hereby absolutely and forever voluntarily consent to any and all official notices and communications to MEC’s members, including those contained in Texas Co-op Power. I acknowledge and agree that such official notices and communications include, without limitation: (1) all notices of member meetings and matters to be considered, or voted or acted upon at all meetings of the members of MEC and (2) changes or proposed changes to MEC’s tariffs, rates, terms and conditions of service, fees, and policies.
  5. I acknowledge and agree that MEC has thoroughly informed me of the full range of notices and other information that I may no longer have access to as a result of the actions described herein.
  6. I do hereby voluntarily and completely waive, release, discharge and hold harmless MEC and its directors, officers, agents, employees, and representatives and all others from any and all responsibilities or liability from harm or damages whatsoever resulting directly or indirectly from my request to no longer receive official member notices and communications from MEC.
  7. I acknowledge and affirm that I have carefully read and assume full responsibility for each of the provisions of this Waiver and Release and that I understand each of the matters set forth in this Waiver and Release.
